     Hello,  My name is Lauren and I am with the Questionnaire DesignResearch Lab at the National Center for Health Statistics (NCHS) inHyattsville, MD.  We are designing a questionnaire for a survey that willbe administered to the US population to determine the health of teens. To do this, we “test” the questions to make sure everyoneunderstands the questions the same way so that we are able to collect the datathat we think we are getting.  Hence, we are looking for parents andteens to answer a variety of health questions.  We want to talkface-to-face to parents or guardians of teens 12 to 17 years of age whomay have difficulties in the following areas: seeing, hearing, walking,learning, behavior, depression or anxiety. The interview with the parent andthe interview with the teen will be no longer than 60 minutes. Theparent/guardian and the teen will receive $40 each.  Interested parents can call in at 301-458-4676
